Daily Health ScreeningProcess & Forms
In an effort to continuously serve members in a safe environment, all participants must complete a daily health screening.
All children, staff, and caregivers must self-screen at home daily. Self-screening shall include fever (temperature of 100.0ºF or above is considered a fever), cough, shortness of breath, gastrointestinal symptoms, new loss of taste/smell, muscle aches, or any other symptoms that may feel like a cold. Children, staff, and caregivers who enter the building must complete a daily health screening questionnaire.
To speed up your arrival process you may choose to go paperless by completing the form online beginning September 21st. A paper copy of this health screening will also be available upon arrival if preferred.
If you choose to complete your health screening online:
- Responses must be submitted daily AFTER you/your child have woken up each morning and you self-screen at home. Responses submitted the night before will not be accepted.
- You will be contacted if your form will not be accepted.
- Specific programs may have time restrictions on when forms must be submitted.
