Daily Health Screening
Process & Forms

In an effort to continuously serve members in a safe environment, all participants must complete a daily health screening.


All children, staff, and caregivers must self-screen at home daily. Self-screening shall include fever (temperature of 100.0ºF or above is considered a fever), cough, shortness of breath, gastrointestinal symptoms, new loss of taste/smell, muscle aches, or any other symptoms that may feel like a cold. Children, staff, and caregivers who enter the building must complete a daily health screening questionnaire.


To speed up your arrival process you may choose to go paperless by completing the form online beginning September 21st. A paper copy of this health screening will also be available upon arrival if preferred.  


If you choose to complete your health screening online:


  1. Responses must be submitted daily AFTER you/your child have woken up each morning and you self-screen at home. Responses submitted the night before will not be accepted.
  2. You will be contacted if your form will not be accepted.
  3. Specific programs may have time restrictions on when forms must be submitted.
